Better Futures workers engage early with young people in care and their support networks, including carers, case managers and care teams. A Better Futures worker provides secondary consultation to the care team, lending expertise to the transition planning process which may include assisting young people to plan for and meet their goals for living arrangements, education, employment, health and well-being and community connections.
WCIG acts as the single point of Common Intake, working with partnered organisations to deliver Better Futures to Melbourne’s Western and Brimbank Melton areas.
Home Stretch is a ground-breaking initiative that helps every Victorian young person in care with support to transition to adulthood and a place to call home until their 21st birthday.
Home Stretch funding can include an accommodation allowance, case work support and flexible funding to facilitate the young person’s access to education, employment and health and wellbeing supports. For more information on Home Stretch, visit: Home Stretch – DFFH Service Providers.
